PNV-57
I got this at a local surplus store, in non-working condition. It came with quite a bit of the kit, however it's missing a few pieces.
I could tell that it had gone through a few hands before mine, and I wasn't the first to try and alter it. At one point, someone had attached an old (80's?) motorcycle headlight that they had painted over to the top, which I removed. Thankfully neither of the tubes were blown, and most of the delicate equipment inside is intact. I had to go through and solder a few wires to what appears to be it's transformer.
I still need to figure out a more permanent power source, as for now I've got it attached to a 9-volt battery clip. I'm not worried too much about authenticity, since these tend to be pretty common (at least outside of the states). It runs through the batteries quickly, and I'd love to get it on something more constant. It's supposed to either attach to a battery pack or connect to a vehicle's cigarette lighter.

There you can see the small toolbox in the bottom open, the bakelite tin of grease in place, and the papers in place.
There's the contents laid out. Record book and paper, grease jar, spare battery(?), cleaning cloth, and a neat little screwdriver. I forgot to add the little metal tool that comes with it in the picture above, so here's a photo of that:
